CHARTER'S CORE

This carter is dedicated to all clients of the Perak Health Department.

1.
FACILITIES
  1.1 Every clients will be appropriately served, regardless of gender, ethnicity, religion or status, socially and economically.
  1.2 Priority will be given to all emergency cases
   
2. SERVICE STATUS
  2.1 Every client will be treated in a fair manner, friendly, considerate, respectfully, courteous, honestly and sincerely.
  2.2 Every client will be given treatment professionally.
  2.3 Every client will be provided with a perfectly clean facilities and environment
   
3. SERVICE INFORMATION
  3.1 Each client may seek clarification and advice on the services provided.
  3.2 Each client will receive a report on a services provided once all regulations are adhere to during the application.
   
4. HEALTH EDUCATION
  4.1 Every client will be brief on the important aspect of health treatment and prevention.

CLIENTS' OBLIGATIONS

To enable Perak Health Department implement the carter effectively, all clients are obliged to :

(i)        Comply with all related laws and regulations ;

(ii)       Practice a healthy life style for the benefit of the community and oneself ;

(iii)      To use all facilities available in a responsible manner.
